Record meaning: not just a world record

A record can be a world record. But more often it's information written down: in medical files, archives, a notebook. And booking a doctor's visit needs another word.

A record is information that's written down and kept: medical records, keep a record. The meaning “best result” exists too: a world record. But a time booked to see a doctor or hairdresser is an appointment, not a record.
